Step 1 — Report it (Form DS-64)

- By mail: Form DS-64 to the address on the form
- Abroad: report at the nearest U.S. embassy/consulate
Step 2 — Replace it
In the United States
You must apply in person with both DS-64 and DS-11 — same process as a first-time application: citizenship evidence, photo ID, photo, $165 total. A police report is helpful but not required.
Abroad
Go to the nearest U.S. embassy or consulate. They can issue an emergency limited-validity passport (often same-day) to get you home, then you replace it with a full passport back in the U.S. Bring any ID you have, a photo if possible, and your travel itinerary.
