Application for Passport: DS-11


Please type or print legibly in black ink when completing all sections of this application.

Do not sign the application until you take the oath administered only by an authorized passport acceptance agent.  To find the nearest authorized passport acceptance facility, click here. Unless authorized as passport acceptance agents, notary publics and attorneys are not allowed to administer or witness your signature on the application.

If you are overseas and need to apply for or renew your U.S. passport, please follow the instructions under the U.S. passport section in Tips for Americans Residing Abroad .

You must use this form if:

  1. You have never had a U.S. passport or
  2. You are under age 16 or
  3. Your most recent passport was issued more than 15 years ago or
  4. Your most recent passport was lost or stolen. (If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64 listed on the left.)

Please follow instructions on pages 1 and 2 and read carefully pages 3 and 4.  Complete both pages of the application and print on one-sided pages (do not sign the application).  Bring pages 3 and 4 of the instructions along with the application when you appear at the acceptance facility or passport agency to apply for your passport.
